Zero sugar, zero calories, all the sweetness. On paper the artificial sweetener is a free lunch, and for a long time that is how it was sold. Then the headlines turned: gut damage, cancer scares, weight gain from a “diet” drink. So which is it? The truth is less dramatic than either side wants, and more interesting. Here is what these sweeteners actually are, why they are everywhere, and what the research does and does not support.

What artificial sweeteners actually are
They are compounds that trigger your sweet-taste receptors with little or no energy behind them. Gram for gram they are hundreds of times sweeter than sugar, so a trace does the job. The usual suspects on a UK label:
- Aspartame (E951). Around 200 times sweeter than sugar. Broken down and digested, not passed through untouched.
- Sucralose (E955). Roughly 600 times sweeter, made from sugar but mostly not absorbed.
- Acesulfame K (E950). About 200 times sweeter, often blended with sucralose or aspartame.
- Saccharin (E954). The oldest one, around 300 to 400 times sweeter, the original “pink packet.”
- Steviol glycosides (stevia, E960) and erythritol (E968) sit in a separate box. They come from plants or fermentation and get marketed as “natural.” Worth knowing, because most of the concern below is aimed at the synthetic four, not stevia or monk fruit.
Every one on that list has passed safety review by the major regulators, and the acceptable daily intakes are set well above what a normal person consumes. When you read “artificial sweeteners are dangerous,” start from there: at ordinary doses, acute toxicity is not the argument. The interesting questions are subtler.
Why they are in almost everything
Sugar is expensive to your waistline and to your teeth, and food companies know shoppers now read labels. Swap sugar for a few milligrams of sucralose and you keep the sweet hit, drop the calories, extend shelf life, and print “no added sugar” on the front. That is why they turn up not just in diet cola but in yoghurts, protein bars, chewing gum, cordials, sauces and half the “healthy” snacks in the aisle. The incentive is commercial before it is nutritional.
The gut microbiome question
This is where the science gets genuinely interesting rather than scary. Your gut bacteria are not passive, and some sweeteners appear to talk to them.
A 2022 controlled trial in Cell (Suez and colleagues) gave healthy adults sachets of saccharin, sucralose, aspartame or stevia for two weeks. Saccharin and sucralose measurably shifted the make-up of the gut microbiome, and in some people nudged blood-glucose responses in an unhelpful direction. Aspartame and stevia did far less. That is a real, well-run study, and it is the strongest signal we have that these are not simply inert.
The honest caveats matter though. It was two weeks, in a small group, and the effect varied a lot person to person. “Changed the microbiome” is not the same as “harmed your health.” We do not yet have the long trials linking that shift to disease outcomes. So the fair reading is: sucralose and saccharin are not biologically silent, the direction of the signal is a little concerning, and the size of the real-world consequence is still unknown.
The metabolism and appetite argument
The bigger, older worry is that a sweet taste with no calories confuses the system. The idea: your body tastes sweet, braces for sugar, gets none, and learns to mistrust the signal, so you overeat elsewhere. It is a tidy theory.
The evidence is a muddle, and I will not pretend otherwise. Large observational studies do link diet-drink consumption to higher weight and worse metabolic markers, but observation cannot untangle cause from effect. People already struggling with their weight reach for diet drinks, which makes the drink look guilty when it may just be a bystander. When you run the randomised trials, where people are actually assigned to swap sugar for a sweetener, the results usually go the other way: modest weight loss or no change, not gain (a 2022 Rios-Leyvraz review for the WHO found much the same). So the “diet drinks make you fat” claim is far weaker than the internet believes.
That said, in 2023 the WHO advised against using non-sugar sweeteners for weight control, concluding they do not help long term and may carry risks. Read that carefully. It is not “they are poison.” It is “swapping sugar for sweetener is not a weight-loss strategy, so do not count on it.” Fair, and worth taking on board.
Cravings, and the taste you are training
Here is the part I find most convincing from lived experience rather than a lab. Whether or not a sweetener spikes your insulin, it keeps your palate wired for intense sweetness. If every drink and snack is aggressively sweet, a plain apple tastes of nothing and unsweetened tea tastes like punishment. You are training your baseline.
When I cut sweeteners for a stretch, the shift is not dramatic on day one, but after a couple of weeks ordinary food tastes sweeter and the pull towards a sugary or “diet” hit fades. That is not a metabolic claim I can hang a citation on. It is a behaviour one, and behaviour is where most of us actually win or lose the nutrition game.
The cancer question, kept in proportion
In 2023 the WHO’s cancer agency (IARC) classified aspartame as “possibly carcinogenic to humans,” group 2B. That headline did a lot of damage and very little informing. Group 2B is the same bucket as aloe vera and pickled vegetables, and it describes how suggestive the evidence is, not how risky the substance is. In the same breath, the safety committee kept aspartame’s acceptable daily intake unchanged: about 40 mg per kilo of bodyweight. For a 70 kg adult that is roughly 14 cans of diet drink a day, every day, before you approach the limit. Unless that is your life, aspartame is not the thing to lose sleep over.
So, are they actually bad for you?
Here is the honest ledger. At normal intakes they are not acutely toxic, and the cancer scare is overblown. The real, defensible concerns are narrower: sucralose and saccharin can shift your gut bacteria in ways we do not fully understand yet, sweeteners are not a reliable tool for losing weight, and a constant sweet signal keeps your palate hooked on sweetness. None of that is a five-alarm fire. All of it is a decent reason not to build your diet around them.
What I actually do: I treat them as an occasional bridge, not a staple. A diet drink now and then over a sugary one, fine. Sweetened everything, all day, is a habit I would rather not keep, less because I fear the molecule and more because I would rather not train my taste to need it.
